Qualifications

  • 4 years of experience in a social service field or an Associate Degree plus 2 years of experience.
  • Bachelor’s degree in a social science field plus at least 6 months experience.

Starting Compensation:

22.36 - 27.95 per hour

Working Location:

San Leandro: San Leandro Center (In-person position)

Working Hours/Shift:

Monday-Friday, AM shift

What you bring to SBHG (qualifications):

  • 4 years of experience in a social service field OR
  • Associate Degree plus 2 years of experience in a social service field OR
  • Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service
  • Valid California Driver's License

How you will make a difference (job overview):

This position has primary responsibility for case management services to clients and their families referred through Stars’ Transition Age Youth (TAY) Program (TAY)

Division/Program Overview:

  • Outpatient, community-based program that serves youth age 18-25 with serious and persistent mental health issues
  • Program provides intensive case management, outreach and engagement, medication support, as well as, mental health counseling, peer groups, and supportive services to assist TAY transition to adult life.
  • Services offered are trauma informed and culturally responsive
