Job summary

Aging True in Florida is seeking a dedicated case manager to assess clients’ demographics, mental and physical health, nutrition, health conditions, caregiver status, social resources and environment, and to develop comprehensive care plans linking needs to resources across Duval County and the broader community.

The role reports to the Duval Office with caseloads primarily in Duval and Clay Counties; requires a Bachelor's degree in social work, sociology, psychology or related field, a valid

Qualifications

  • Bachelor’s degree in social work, sociology, psychology, or related field.
  • Valid Florida driver’s license and ability to use personal vehicle.
  • Excellent communication, organization and computer skills.
  • Background checks: Level 2 background screening, drug testing, and NSOPR screening.
  • Knowledge of Medicare/Medicaid programs a plus.

Responsibilities

  • Assesses client need for services through home visits using the DOEA 701B.
  • Develops a care plan (DOEA Form 203A) to meet service needs identified in the DOEA 701B.
  • Arranges and develops a formal agreement for the provision of services through a Service Purchase Order (SPO).
  • Serves as the coordinator for all delivered services and monitors service delivery to assess progress and need for change.
  • Arranges support services for caregivers when appropriate and counsels them regarding caregiver stresses.
  • Documents case history in the client case file and maintains up-to-date files with the case aide.
  • Reassesses the client through phone calls and semi-annual home visits and updates the care plan with client consent.
  • Terminates clients when needs are met or services are no longer appropriate; completes weekly ServTracker billing.

The position provides potential and existing clients with a thorough assessment of their current situation (demographics, mental and physical health, nutrition status, health conditions, caregiver status, social resources, and environment). The case manager develops, along with the client and/or caregiver, a comprehensive care plan that links the specific needs of the client to the appropriate resources (both formal and informal), with the agency itself and throughout the Duval County community. The needs of the client are monitored as required by the State Department of Elder Affairs. Care plans are modified as needed to meet the changing needs of the client.

This position will report to our Duval Office and assigned caseloads will be predominately in Duval and Clay Counties.

Qualifications/Requirements:
  • Bachelor’s degree from an accredited college or university in social work, sociology, psychology, or other related fields.
  • Must have a valid Florida driver’s license.
  • Must use personal vehicle to drive to clients' homes.
  • Must have excellent communication, organization and computer skills.
  • Previous experience in social work training and/or gerontology a plus.
  • Knowledge of Medicare and Medicaid programs a plus.
  • Requires a valid Florida driver's license, auto insurance and use of personal vehicle.
  • Must submit to drug testing and National Sex Offender Public Registry screening.
Essential Functions:

  • The person in this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc.
  • Must be able to remain in a stationary position 50% of the time.
  • Requires sitting at a computer for up to 8 hours.
  • The person in this position frequently communicates with clients who have inquiries or questions. Must be able to exchange accurate information in these situations.
  • Frequently works in outdoor weather conditions. Frequently in client home settings.
