Job summary

TASC (Treatment Alternatives for Stronger Communities) is seeking a compassionate Case Manager to support clients involved in the criminal legal system who face substance use and behavioral health challenges. You will perform assessments, referrals, and case management to promote recovery and stability within the ADult Criminal Justice and Treatment Services division.

Starting at 44,676 to 47,650 dollars annually, compensation varies with experience and education.

Qualifications

  • High school diploma or GED.
  • Valid driver's license.
  • CADC certification within two years of employment may be required based on contractual obligations.
  • Must be able to pass an IDOC Background check.
  • Proficiency in Microsoft Office suite.
  • Experience in group facilitation.
  • Knowledge of ASAM criteria, DSM-V related to substance use, and Code of Ethics for substance abuse professionals or ability to acquire.

Responsibilities

  • Conduct comprehensive assessments and develop service plans tailored to client needs.
  • Facilitate intake and enrollment into TASC programs.
  • Provide referrals and crisis intervention for behavioral health, housing, employment, and other supports.
  • Collaborate with community partners and internal staff to ensure coordinated care.
  • Apply DSM-5 and ASAM criteria in evaluating client needs and treatment referrals.
  • Maintain timely and accurate documentation, including progress notes, reports, and discharge summaries.
  • Monitor progress toward service goals and ensure compliance with all reporting systems.
  • Participate in staff meetings, trainings, and multi-disciplinary team discussions.
  • Support clients in navigating systems and accessing essential services.

We are currently looking for full-time – Case Manager

Starting at $44,676 - $47,650; contingent upon experience, education, etc.

Position Overview:

TASC is seeking a compassionate and motivated Case Manager to support clients involved in the criminal legal system who are struggling with substance use and behavioral health challenges. Case Managers in our Adult Criminal Justice and Treatment Services (ACJS) division work closely with individuals to provide assessments, referrals, and case management services that support recovery and long-term stability

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Conduct comprehensive assessments and develop service plans tailored to client needs
  • Facilitate intake and enrollment into TASC programs
  • Provide referrals and crisis intervention for behavioral health, housing, employment, and other supports
  • Collaborate with community partners and internal staff to ensure coordinated care
  • Apply DSM-5 and ASAM criteria in evaluating client needs and treatment referrals
  • Maintain timely and accurate documentation, including progress notes, reports, and discharge summaries
  • Monitor progress toward service goals and ensure compliance with all reporting systems
  • Participate in staff meetings, trainings, and multi-disciplinary team discussions
  • Support clients in navigating systems and accessing essential services
Our Ideal candidate:
  • High school diploma or GED equivalent
  • Valid driver's license
  • CADC certification within two years of employment with TASC may be required based on contractual obligations
  • Must be able to pass an IDOC Background check
  • Demonstrated proficiency in the Microsoft Office suite
  • Demonstrated experience in group facilitation
  • Knowledge of the current ASAM placement criteria, DSM-V as it relates to substance-related disorders, and Code of Ethics for substance abuse professionals, or the ability to acquire the knowledge
  • Comfort working inside a correctional setting and with persons convicted of serious violent offenses
  • Flexibility in thinking and approaches to leading group discussions
  • Good verbal, written, and interpersonal communication skills
  • Good time management skills with a proven ability to meet deadlines
