Job summary

Treatment Alternatives for Stronger Communities (TASC) is seeking a full-time Case Manager to provide comprehensive case management for adults and youths with behavioral health needs statewide, with referrals to services. The role involves direct service delivery, coordination with health and community resources, and documentation of progress.

The incumbent will collaborate with partners and participate in staffing and reporting.

Qualifications

  • Bachelor’s degree required in Counseling, Social Work, Psychology, Criminal Justice, or related field; Master’s degree preferred.
  • Bi-lingual English/Spanish required or QMHP per contract obligations.
  • Ability to obtain CADC/CARS certification within the first two years in the field.

Responsibilities

  • Assist individuals in achieving service plan goals through direct services and coordination with behavior health, public agencies, and community resources.
  • Assess, evaluate, facilitate, and monitor progress toward established goals.
  • Complete intake packets for program enrollment and participation.
  • Provide referrals and crisis intervention for identified needs.
  • Interview individuals, review records, and confer with professionals to review progress.
  • Collaborate with internal and external partners and participate in staff meetings.
  • Identify and refer individuals with behavioral health needs.
  • Submit internal and external program reports and maintain all documentation.

We are currently looking for full-time – Case Manager

Division Overview

The Adult Criminal Justice and Treatment Services (ACJS) supports clients who may have struggles related to a substance use disorder. We provide opportunities for clients to rebuild their lives so they can become healthy and self-sufficient. ACJS’ work includes screening and assessments for substance use disorders, placing clients into treatment programs, monitoring and reporting on progress, and providing support including, but not limited to, insurance enrollment, obtaining a state ID, and finding employment resources.

Summary

TASC Case Manager will provide comprehensive and specialized case management services for adults and youths with identified Behavioral Health needs referred to TASC from multiple systems. TASC Case Manager will provide direct services and referrals based on identified needs of the individuals enrolled into TASC services. TASC provides case management services Statewide and location is determined by work site location.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources. (Approximate 75% of time spent in this area)

Assess, evaluate, facilitate, and monitor individuals’ progress in relationship to established goals and objectives

Complete intake packets for program enrollment and participation

Provides referrals and crisis intervention for the individuals’ identified needs

Interview individuals, review records, and confer with other professionals to review individual progress, and to determine their suitability for program participation

Knowledge of DSM V and ASAM criteria

Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services.

Identify and refer individuals with behavioral health needs

Complete and submit internal and external program reports

Complete and maintain all required individual and program related documentation which includes, documenting the results of screenings, enrollments, service plans, reports, progress notes, discharge summaries and other individual-related activities including accurate reporting of services

Monitor all individuals determined by individual progress towards service goals

Monitor TASC database system and contractual/external database systems to ensure Individual enrollment

  • Bachelor’s degree required in Counseling, Social Work, Psychology, Criminal Justice, or related field or Master’s Degree required, must be bi‑lingual in English/Spanish, or classified as a QMHP as required by contract obligations
  • Ability to obtain CADC/CARS certification within the first two years in the field.
  • Demonstrated proficiency in the Microsoft Office suite
  • Demonstrated experience in group facilitation
  • Certain programs may require a higher level of education, certification and/or licensures based on contractual obligations
  • Ability to support a culture of diversity, equity, and inclusion.
  • Ability to take initiatives and have creativeness
  • Flexibility in thinking and approaches to leading group discussions
  • Excellent verbal, written, and interpersonal communication skills.
  • Excellent time management skills with a proven ability to meet deadlines
SUPERVISORY RESPONSIBILITY

This position has no supervisory responsibilities.

The agency currently offers hybrid work schedules that combine in-office and remote work. Employees are required to report to the work site three days per week, with the option to work from home to two days remotely after 60 days of employment contingent upon job duties.
