Personalized Nursing LIGHT House in Plymouth seeks responsible Case Managers to locate care and resources for clients and transport them to approved providers. Ideal candidates have experience with substance use disorders and strong communication skills.

Responsibilities include initiating case management services, coordinating with providers, and maintaining a welcoming environment for clients and families. A Bachelor's in related fields and valid driving license are required.

Qualifications

• Bachelor's degree in social work, psychology, criminal justice, etc.
• CAADC, CADC or Development Plan from MCBAP is required/preferred.
• Driver's License and a clean driving record; comfortable driving clients in personal vehicle.
• Knowledge of Microsoft Office (Word, Excel, PowerPoint).
• Familiar with substance use disorders and mental health disorders; provide welcoming environment.
• Able to multi-task; confidential and good listener; effective written and verbal communication.

Personalized Nursing LIGHT House is a premier behavioral health care provider with 34 years of specialized addiction treatment service serving the adult population. We have locations in Plymouth, Canton, Brighton, & Madison Heights. Our mission is to be an innovative and effective behavioral healthcare corporation (utilizing the LIGHT model of care), that strives for excellence clinically as well as administratively. Specifically, our intent is to increase client well-being, decrease addiction severity, improve the quality of clients' mental and physical heath, and contribute positively to society. All services will integrate substance use disorder, mental health, and primary health care treatment in cohesive and client-centered programs.

POSITION SUMMARY:

We are seeking responsible and compassionate Case Managers to help locate applicable care and resources for our clients and to transport clients to/from approved resource providers. The ideal candidates will take initiative in instituting case management services and communicating with clients in effective ways. Experience with persons with substance use disorder and/or co-occurring disorders is preferred.

REQUIREMENTS:
