Job summary

Alma Family Services in California seeks a case manager to monitor clients’ treatment adherence, coordinate with local agencies, and provide supportive services in office or field settings. You will assist the primary therapist, participate in supervision, and maintain records and reports.

The role requires a BA degree with 1 year of relevant experience, strong communication, and ability to work with culturally diverse populations.

Qualifications

• BA degree and 1 year relevant experience or equivalent experience.
• Effective written and oral communication skills.
• Experience with culturally diverse populations preferred.
• Previous case manager experience preferred.
• Bilingual English/Spanish required.
• Must have reliable transportation, valid CA driver’s license, auto insurance, and acceptable driving record.

• Monitor clients’ compliance with treatment as prescribed by the treatment team.
• Establish coordination with local agencies in the provision of services.
• Provide supportive services, in the office or in the field, including gathering medical and social histories, screening referrals and linking clients with other agencies as necessary.
• Assist primary therapist in carrying out linkage, advocacy and services.
• Participate in agency supervisor/consultation sessions as appropriate.
• Maintain agency records and prepare reports as required.

Able to utilize sound judgment and deescalate unplanned situations without immediate supervision. Able to work independently and as part of a team. Bilingual English/Spanish required. Must have reliable transportation, valid CA driver’s license, auto insurance, and acceptable driving record.

Salary: $22 - $25/hour
