Alma Family Services in California seeks a case manager to monitor clients’ treatment adherence, coordinate with local agencies, and provide supportive services in office or field settings. You will assist the primary therapist, participate in supervision, and maintain records and reports.
The role requires a BA degree with 1 year of relevant experience, strong communication, and ability to work with culturally diverse populations.
Help monitor clients’ compliance with treatment as prescribed by treatment team. Establish coordination with local agencies in the provision of services. Provide supportive services, in the office or in the field, to include gathering medical and social histories, screen referrals and make appropriate client linkages with other agencies as necessary. Assists primary therapist in carrying out, linkage, advocacy and services. Participate in agency supervisor/consultation sessions as appropriate. Maintain agency records and prepares reports as required.
BA degree and 1 year relevant experience or equivalent experience. Effective written and oral communication skills. Experience, knowledge, skill and abilities with culturally diverse populations preferred. Previous case manager experience preferred. Able to utilize sound judgment and deescalate unplanned situations without immediate supervision. Able to work independently and as part of a team. Bilingual English/Spanish required. Must have reliable transportation, valid CA driver’s license, auto insurance, and acceptable driving record.
Salary: $22 - $25/hour
