Job summary

High Plains Mental Health Center is seeking a Case Manager in Phillipsburg, KS. The role involves supporting adults with mental illness and youth, building skills and independence, and coordinating individualized treatment plans.

You will travel across our 20-county service area, visit clients at home, school, or in the community, and participate in group activities to enhance life skills and overall well-being.

Qualifications

• Experience counts! equivalent work in human services or a BA/BS degree.
• Strong communication and empathy are a must.
• Comfortable in a variety of human service settings.
• Able to pass drug screen and be subject to a random drug testing pool.
• Must have valid drivers license.

• Collaborate with clients and families to develop and implement individualized treatment plans.

What You’ll Do

Support individuals who struggle with their mental health including adults with severe and persistent mental illness and emotionally disturbed youth in building skills, increasing independence, and working toward meaningful personal goals.

Serve individuals with Severe and Persistent Mental Illness & Severely Emotionally Disturbed youth, as well as other assigned individuals requiring case management services.

Collaborate with clients and families to develop and implement individualized treatment plans that address mental, emotional, and social needs.

Travel throughout our 20-county service area to meet clients where they areat home, in schools, or in the community (Center vehicle provided).

Lead or assist in group activities that focus on building life skills, managing symptoms, and improving overall quality of life.

Advocate for your clients, helping them navigate systems and access essential community resources such as housing, education, healthcare, and employment support.

Maintain clear, timely, and accurate documentation to track progress, support quality care, and meet compliance standards.
