Job summary

High Plains Mental Health Center in Phillipsburg, KS is seeking a Case Manager to support individuals with mental health needs across our 20-county service area. This full-time, in-person role offers on-site training, a starting wage above $22 per hour, incentive pay, and a $500 sign-on bonus.

You will work Monday–Friday, 8:00 AM–5:00 PM with rotating on-call, travel as needed to meet clients at home, school, or in the community. Center vehicle is provided.

Qualifications

• Experience in human services or BA/BS degree considered equivalent
• Strong communication and empathy are required
• Able to pass drug screen and be in random pool
• Valid driver’s license required

Responsibilities

• Support individuals with mental health challenges to build skills and independence
• Serve individuals with severe mental illness or emotionally disturbed youth
• Collaborate to develop and implement individualized treatment plans
• Travel across a 20-county service area with center vehicle provided
• Lead or assist in group activities to build life skills
• Advocate for clients to access housing, education, healthcare and employment resources
• Maintain thorough, timely documentation for quality care and compliance

What You’ll Do:

• Support individuals who struggle with their mental health—including adults with severe and persistent mental illness and emotionally disturbed youth—in building skills, increasing independence, and working toward meaningful personal goals.
• Serve individuals with Severe and Persistent Mental Illness & Severely Emotionally Disturbed youth, as well as other assigned individuals requiring case management services.
• Collaborate with clients and families to develop and implement individualized treatment plans that address mental, emotional, and social needs.
• Travel throughout our 20-county service area to meet clients where they are—at home, in schools, or in the community (Center vehicle provided).
• Lead or assist in group activities that focus on building life skills, managing symptoms, and improving overall quality of life.
• Advocate for your clients, helping them navigate systems and access essential community resources such as housing, education, healthcare, and employment support.
• Maintain clear, timely, and accurate documentation to track progress, support quality care, and meet compliance standards.

Requirements

• Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
• Great with people—strong communication and empathy are a must.
• Comfortable in a variety of human service settings.
• Able to pass drug screen & be subject to a random drug testing pool.
• Must have valid driver’s license.
