Petaluma Post-Acute Rehab in Petaluma, CA is seeking a Nursing Case Manager (LVN or RN) to coordinate nursing and rehabilitation services for residents. You will take ownership of admissions, work with the IDT and Directors of Nurses, and advocate for residents with payers and medical management professionals.

The role emphasizes collaboration with care staff, discharge planning, and ensuring seamless, high-quality patient experiences in a premier nursing facility.

Qualifications

• Active LVN or RN license required.
• Prior experience in skilled nursing preferred but not required; related case management experience acceptable.
• Excellent written and oral communication skills.
• Ability to multi-task and treat everyone professionally.
• Works collaboratively and independently, follows procedures and suggests improvements.
• Emotional maturity; remains calm under stress.

Responsibilities

• Greet new patients/residents to ensure a positive admission experience.
• Evaluate and communicate resident status and care plan changes to payers.
• Document all payer interactions regarding resident progress and instructions.
• Meet with the interdisciplinary team to coordinate services for total care.
• Coordinate with business office and medical records to ensure census accuracy and payment.

Position Summary

The Case Manager is responsible for coordinating the delivery of Nursing and Rehabilitation services to our residents in collaboration with the facility’s care staff. Takes ownership of all admissions as they enter into the facility, communicating with the resident/RP. Works very closely with the IDT and Director of Nurses. The case manager is to serve as patient advocate and liaison to insurance carriers and other medical management professionals, and is in charge of managing the nursing portion of our resident care conferences. Works closely with IDT and Social Services to assist in planning and executing a safe discharge.

Qualifications

• Have an active LVN or RN license
• Prior experience in skilled nursing preferred but not required (if not, have you work in a field related to case management?)
• Outstanding communication (oral and written) skills
• Ability to multi-task and treat everyone at work kindly and professionally
• Works collaboratively and independently, following procedures and guidelines, making recommendations for improvements
• Completes tasks on time, reporting up on successes, challenges, and offering solutions to problemsEmotional maturity – ability to remain calm and professional under stress

Responsibilities

• Meet with interdisciplinary team to coordinate services ensuring resident’s total regimen of care is maintained
• Coordinate and communicate with business office and medical records to ensure accurate census and payment of Managed care and Medicare residents
