The Avenues Transitional Care Center in San Francisco, CA is seeking a Licensed Vocational Nurse (LVN) to provide and oversee patient care in a premier nursing facility, ensuring safety and compliance with regulations. The role emphasizes observation, medications, vital signs, treatment prep, and collaboration with CNAs; you will report changes to DNS/Administrator, and help maintain care plans.

Qualifications

• Must hold valid LVN license in good standing with the State Board of Nursing.
• Two+ years in long-term care preferred.
• CPR and First Aid certification required.
• Strong communication skills and a calm, professional demeanor.

Responsibilities

• Observes, reports and documents/charting on patient conditions.
• Administers medications in accordance with physician orders.
• Takes and records temperatures, respiration and pulse.
• Sets up treatment trays, prepares instruments and/or other equipment, as needed for treatments.
• Maintains working knowledge of assigned patient’s conditions at all times.
• Reviews reports from previous shifts to determine changes in status of residents and assigns duties to CNAs or other nursing personnel.
• Turns over accurate records and ensures oncoming shift nurses are updated.
• Notifies DNS and/or Administrator of changes in patient condition or other job-related problems.
