Job summary

St. Joseph's Health is seeking a clinician to collaborate with the medical team to develop and implement a plan of care, coordinating all required services and ensuring patients access to eligible programs.

This on-site role requires a Bachelor's degree in Social Work, Psychology, Psychiatric Rehabilitation or related mental health field. Bilingual (Spanish/English) preferred; the candidate should be able to counsel and educate patients and their families and resolve problems using mental health
