Job summary

Steele County is seeking a dedicated Case Manager to provide professional social services across child protection, vulnerable adults, mental health and related areas. You will develop treatment plans, coordinate services, and collaborate with courts, law enforcement and healthcare professionals to ensure safe, effective client outcomes.

The role requires a Bachelor’s degree in behavioral or social sciences, social work, or a closely related field and a valid driver’s license.

• Assist clients in accessing county, state, and federal programs; coordinate ongoing service delivery
• Work collaboratively with law enforcement, courts, medical professionals, community agencies and internal departments

Requirements

Under the direction of the Supervisor, the Case Manager is responsible for providing professional social services to clients and families in a wide variety of program areas including child protection/welfare, child services, vulnerable adults, mental health, developmental or physical disabilities, chemical dependency, child care and elderly services. Duties and activities include but are not limited to formulating client treatment/service plans; conducting case management activities; performing crisis intervention; referral services; assessment and family group conferences; and other social service activities in accordance with program requirements, regulations, statutes, mandates, department guidelines, procedures and data privacy requirements. Duties include but are not limited to:

• Review and assess reports of abuse, neglect, and safety concerns for children and adults.


• Conduct child protection, vulnerable adult, foster care, and home study assessments.


• Provide crisis intervention for situations involving mental health emergencies, domestic violence, unsafe housing, and unmet basic needs.


• Develop individualized service and detailed care plans; monitor progress and adjust plans as needed.


• Assist clients in accessing county, state, and federal programs; coordinate ongoing service delivery.


• Work collaboratively with law enforcement, courts, medical professionals, community agencies and internal departments.


• Prepare documentation, case records, court reports, treatment plans, and licensing materials in accordance with regulations.


• Testify in court and participate in guardianship, conservatorship, and civil commitment processes.


• Support foster home licensing, recruitment, and training efforts.


• Maintains regular contact and communication with clients, providers, and community partners.


• Maintain professional knowledge through required trainings, policy updates, and continuing education.


• Provide mentoring and support to new staff.


• Performs other duties of a comparable level or type, as required.

• Steele County will contribute to HSA/VEBA $1,700 for single or $3,500 for family (if you elect to pay for our Medical insurance that is provided by Medica); other benefits that are available include dental, vision, life insurance, long term disability insurance, deferred compensation plan, pension plan, employee assistance program, paid time off, etc.


• Cost of living adjustment and pay increases per union contract. The pay range listed is our 2026 rates. We are currently set to receive a 4% cost of living adjustment on 1/1/2027.
