Who We Need

As a member of the Integrated Care Team, you will participate in developing the comprehensive assessment and Integrated Service Plan (ISP), and implement services according to the plan. You will coordinate with healthcare, behavioral health, and wellness providers, maintain documentation on members’ progress, and support individuals in achieving their recovery goals.

Minimum Qualifications:
  • High School degree required, with a minimum of four (4) years of experience in behavioral health and SMI, OR
  • Associates degree in a related field with two (2) years of related experience OR
  • Bachelor's degree in a related field with one (1) year of related experience.

Case Managers must have the ability to transport members using their own vehicle, possessing:

  • A valid Arizona (AZ) driver's license
  • A clean driving record
  • Current auto insurance and registration

Must have a Level 1 fingerprint clearance card with no restrictions issued by Arizona Department of Public Safety or be eligible to receive one within eight (8) weeks of hire.

Safety sensitive roles include tasks that could affect the safety or health of the employee or others. Bilingual (English/Spanish) skills are preferred.

Job Summary (Including, but not limited to)
  • Collaborate with the care team to engage, educate, communicate, and coordinate care with service participants, families, providers, and community resources, ensuring all services in the ISP are implemented.
  • Develop, maintain, and manage the ISP within required timeframes.
  • Participate in developing assessments and the ISP.
  • Implement services, provide ongoing care, and review progress.
  • Coordinate with healthcare providers, document progress, and assist individuals in achieving recovery goals.
  • Additional duties may be assigned to meet program needs.
