POSITION SUMMARY RN Case Manager for hospital care coordination-discharge planning. POSITION DUTIES

• EMR: EPIC - Do you require InterQual criteria or Milliman experience: InterQual - Is this position for UR, discharge planning, or both?: Acute Care Coordination (Inpatient nursing units, potential for ED and Maternity coverage) - What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.): Inpatient nursing units, potential for ED and Maternity coverage - Experience or 'Must have' skills: MCG/InterQual reviews, Acute Care Coordination discharge planning experience - Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?): Mon-Fri 8-1630 - Weekend Requirements: No - Holiday Requirements: No - How many shifts of unit/department specific orientation?: 3-5 days - Additional Notes: Fast paced turn around for discharging patients. Attributes that will help someone be successful in this position include communication, time management, organizational skills, leadership, adaptability, problem solving
