Medical Center Health System seeks a Case Manager Nurse to lead the interprofessional team in continuity of care and discharge planning. The Care Coordinator will facilitate coordinated patient services during hospitalization to ensure a smooth transition.
The role requires a current RN license in Texas and 1–3 years of acute care plus case management experience, with knowledge of health plan products and community resources. Strong assessment and communication skills are essential.

The Care Coordinator facilitates integrated and comprehensive patient services during hospitalization. The Care Coordinator provides ongoing assistance and consultation to the healthcare team through a comprehensive assessment of patient needs, planning and implementation of an interprofessional care plan, and communication of individual patient needs. Conducts initial, concurrent, and retrospective review functions in support of the hospital case management program and makes appropriate referrals to designated Physician Advisors.
