Fort Duncan Regional Medical Center in Eagle Pass, TX is seeking a skilled RN Case Manager to coordinate care across departments and ensure patients receive appropriate services based on clinical needs and safety standards.
The role requires hospital case management experience and Texas RN licensure, with knowledge of Medicaid/Medicare guidelines to support discharge planning and care transitions within a collaborative multidisciplinary team.
Coordinates with other departments to provide maximum level of services. Must be able to demonstrate the knowledge and skill necessary to provide care, based on physical, psychosocial, educational and safety related criteria, appropriate to the age of the patients served in his / her assigned service area. The skills and knowledge needed to provide such care may be gained through education, training and / or experience.
