JOB SUMMARY:

Under general supervision, is responsible for the development and implementation of the Individualized Parent Training Plans for individuals and their advocates in accordance with the regulations and requirements of the applicable governmental and other regulatory agencies. Provides case management services to assigned individuals and determines individual aptitudes, interests and preferences. Identifies appropriate service providers and links individuals and providers. Coordinates services and monitors individual’s satisfaction with the services.

COMPENSATION: $27.81/hourly + Industry Leading Benefits

The position is grant funded through September 30, 2026.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Provides social services to each person supported direct toward maximizing his/her social functioning, enhancing coping capacity of his/her family, asserting and safeguarding his/her human and civil rights and those of his/her family and fostering their human dignity and personal worth.
  2. Develops and monitors individual training plan in accordance with regulatory and agency policy.
  3. Participates in departmental planning. Interprets, documents and follows up on all recommendations of department team meetings.
  4. Includes channels of communications and consultation with staff to interpret and explain parent’s psychological needs as he/she progresses through the program.
  5. Develops goals and activities generated by team. Produces an active treatment plan schedule for each individual served.
  6. Documents comprehensive functional assessment and review of plan of care as needed.
  7. Ensures proper data collection through observation and in-service training.
  8. Makes regular and meaningful family, people supported, and agency-wide contacts.
  9. Trains and instructs staff on individual’s specific goal plans, relevant data collection, and general procedures and practices.
  10. Maintains contacts with government social services agencies, not-for-profit social service agencies to access resources and/or advocate for services.
  11. Ensures that all benefits for which a person supported may be eligible for are applied for in an expeditious manner and to effect transfer of any prior accumulated funds to agency.
  12. Provides systematic follow-up to assure referral to appropriate community agencies upon discharge.
  13. Works regularly with, develops, or is responsible for maintaining security of confidential information such as records of person receiving services which requires regular use of discretion. Improper disclosure may cause embarrassment, disruption, or conflict to agency or persons receiving services and their families.
  14. Coordinates admissions to program including preparation of referral material, initial screening of applicants, contact with families, and other referral services.
  15. Maintains up to date files on applicants.
  16. Writes monthly case notes on all client’s interpersonal, social, workshop progress and family contacts.
  17. Keeps abreast of changes in field.
  18. Performs other related duties as requested.

QUALIFICATIONS:

  1. This position requires an extremely perceptive person, who is capable of relating to individuals at all levels. As situations present themselves, the employee must be sensitive to program needs, employee goodwill, and the public image.

  4. Strong initiative, an eagerness to learn, and the ability to work both as part of a team and independently.
  5. Ability to maintain confidentiality.
  6. Ability to prioritize assigned tasks and travel to, and between programs and administrative locations.

EDUCATION and/or EXPERIENCE:

  1. Must be a qualified intellectual disabilities professional (Bachelor’s degree or higher, plus a minimum of three years experience working with individuals with developmental disabilities).
  2. Must have experience in providing parenting skills training services.
  3. Clinicians must also be familiar with the workings of New York City’s child welfare and I/DD systems.

LANGUAGE SKILLS:

  1. Ability to read, write, speak, and understand English; bilingual Spanish speaking preferred.
  2. Excellent written and verbal communication skills.
