Job Title: RN Care Manager

Compensation: $48 – $68/hour (based on experience)

About the Role

Fusion Solutions is seeking an experienced and compassionate RN Care Manager to join our client’s interdisciplinary team. This is anopportunity offering a competitive hourly rate and the chance to make a meaningful impact on patient care across inpatient units, the Emergency Department and other care areas.

Position Summary

The RN Care Manager is responsible for assessing, planning, coordinating, and monitoring individualized care plans in collaboration with physicians, social workers, and other healthcare professionals. This position is key in ensuring patients receive the appropriate level of care while balancing clinical quality, regulatory compliance, and financial considerations.

Key Responsibilities:

• Develop and oversee comprehensive care plans for patients in collaboration with interdisciplinary teams
• Coordinate patient care across settings, ensuring appropriate level of care and safe discharges
• Evaluate patient progress and adjust care plans based on evolving clinical needs and payer requirements
• Act as a liaison with insurance companies and third-party payers regarding coverage, authorizations, and utilization review
• Utilize screening criteria (InterQual, MCG) and understand payer regulations for optimal care coordination
• Navigate discharge planning processes, ensuring patients are connected with appropriate community resources
• Maintain accurate documentation and reports in compliance with regulatory standards

Required Qualifications:

• Current NYS Registered Nurse license
• Bachelor’s Degree in Nursing (BSN) or equivalent
• ACM (Accredited Case Manager) or CCM (Certified Case Manager) certification required
• Minimum 4 years of recent experience in acute Medical-Surgical, Psychiatric, ED nursing, or prior case management
• Strong knowledge of payer systems, eligibility, reimbursement guidelines, and community resources
• Experience using InterQual or MCG criteria
• Excellent communication, time management, and interpersonal skills
• Ability to multitask, prioritize, and collaborate across teams effectively

Preferred Skills:

• Familiarity with discharge planning regulations
• Strong negotiation and problem-solving skills
• Experience in hospital-based case management
