support services, and paid work experiences to launch participants into meaningful
careers and economic mobility. Leveraging deep industry partnerships and an
employer-driven approach, VANGUARDE is driving systems change to build a diverse
and equitable tech workforce.
Position Overview:
We are seeking a dedicated and compassionate Train & Earn Case Manager to join
our team. The Case Manager will work closely with participants, primarily at-risk youth,
to create individualized Strategic Service Plans (SSPs), monitor their progress, and
connect them with essential resources and services. This position plays a vital role in
supporting participants through their personal development journey while working
collaboratively with our multidisciplinary team, including job developers, outreach
coordinators, mental health specialists, and the program director.
Key Responsibilities:
Develop Individualized Service Plans (ISPs): Work directly with participants to
understand their needs and goals, and develop customized action plans tailored
to their personal and professional development.
Progress Monitoring: Regularly track and evaluate participants' progress
against established goals and milestones, making adjustments to plans as
needed.
Resource Coordination: Connect participants with relevant community
resources, social services, educational opportunities, and employment support
as part of their development.
Weekly Participant Engagement: Meet with each participant once a week to
provide ongoing guidance, emotional support, and encourage goal achievement.
Documentation & Reporting: Maintain accurate and timely documentation of all
interactions, progress updates, and service interventions in participant files and
the Program Tracking System (PTS).
Collaboration: Work closely with the job developer, outreach coordinator, and
mental health specialists to ensure holistic support for each participant.
Advocate for Participants: Act as a liaison between participants and external
resources, ensuring participants receive the services they need in a timely and
supportive manner.
Crisis Management: Respond to any crises or challenges participants may face,
providing immediate intervention or referral to appropriate services.
Qualifications:
Bachelor’s degree in Social Work, Psychology, Human Services, or related
field.
Proven experience working with at-risk youth, navigating critical social services,
and providing support to underserved populations.
Strong knowledge of community resources, social service programs, and the
local network of agencies.
Experience with individualized case planning and progress tracking.
Excellent communication and interpersonal skills; ability to engage and build
trust with youth.
Ability to work independently as well as collaboratively within a multidisciplinary
team.
Strong organizational skills with attention to detail and accurate record-keeping.
Cultural competency and a passion for supporting diverse populations.
Ability to maintain confidentiality and handle sensitive information responsibly.
Preferred Qualifications:
Experience working in a nonprofit setting.
Familiarity with program tracking systems or case management software.
Knowledge of mental health and behavioral health services.
Full-time, in-person role 9am-6pm on-site

Salary & Benefits:
$60,000-$65,000 annually, plus comprehensive benefits including premium-free health Company Description insurance, 401k with employer match, generous paid time off and parental leave.
