Job summary

Ali Forney Center in New York, NY seeks a Transitional Housing Case Manager to provide trauma-informed, client-centered services for LGBTQ+ homeless youth aged 21-24. The role focuses on housing stability, crisis response, and service coordination.

ABOUT THE ORGANIZATION

The Ali Forney Center (AFC) protects LGBTQ+ youth from homelessness and helps them build the tools needed to live independently. Founded in 2002, AFC provides housing, healthcare, and educational support to thousands of youth each year. AFC creates an affirming, supportive environment where LGBTQ+ youth can thrive.

POSITION SUMMARY

The Transitional Housing Case Manager will provide comprehensive case management services to LGBTQ+ homeless youth ages 21-24. This role delivers holistic, trauma-informed, culturally responsive care by assessing ongoing client needs, developing Individualized Service Plans, coordinating with internal and external resources, connecting clients to necessary resources and long term housing to prepare them for independent living.

The primary focus areas include housing stability, crisis response, and service coordination.

WORK SCHEDULE

Monday-Friday 10am-6pm with one late night 12p-8p. Some weekends and holidays required

WORK FLEXIBILITY: 1 remote day per week

KEY RESPONSIBILITIES
  • Responsible for recordkeeping and documentation, maintaining client charts, and compiling statistics for regular program reporting and data entry into contract databases.
  • Facilitate comprehensive assessments of residents’ needs, strengths, and goals.
  • Develop and implement Individualized Service Plans that address housing, mental health, vital documents, legal, entitlement benefits, education, employment, and health needs.
  • Provide crisis intervention and ongoing support.
  • Collaborate with Ali Forney Center’s Residential Treatment Team, Health Services,Mental Health, and other relevant teams to coordinate comprehensive care for residents.
  • Advocate for residents’ rights and access to services.
  • Assist residents in accessing housing resources, employment opportunities, educational programs, and health care services by accompaniment as needed.
  • Participate in regular supervision, training, and professional development activities.
  • Attend and participate in weekly treatment planning meetings, housing staff meetings, supervision, and other relevant meetings for this role.
  • Develop relationships with NYC service providers, including but not limited to those addressing social, educational/vocational, medical, mental health, and housing needs.
  • Responsibilities may change based on organizational needs. Employees may be required to perform other duties as assigned.
Qualifications
QUALIFICATIONS
  • Must have a High School diploma or equivalency required.
  • Computer knowledge and competent writing skills.
  • 6 months-1 year experience in residential housing, supportive housing, shelter, transitional living, or youth development programs.
  • Strong written and oral communication.
  • Knowledge of LGBTQ+ issues, youth homelessness, and relevant social service systems.
  • Ability to collaborate in a diverse environment and build rapport with clients from diverse backgrounds.
  • Ability to manage multiple tasks simultaneously.
  • Basic assessment and counseling skills.
PREFERRED QUALIFICATIONS
  • Bachelor's degree in Social Work, Human Services, Psychology, Public Administration, Nonprofit Management, or other discipline.
  • Bilingual (Spanish or other commonly spoken community languages)
  • Competency in trauma-informed care and harm reduction.
  • Strong organizational skills with attention to detail.
  • Demonstrated knowledge of navigating NYC social service agencies, including the Runaway and Homeless Youth (RHY) system, shelter programs, and transitional housing resources.
  • Ability to work independently as well as collaboratively.
  • Knowledge of psychosocial needs of LGBTQ+ and homeless population.
  • Ability to use critical thinking and problem solving.
  • Experience working with LGBTQ+ youth
  • Experience working within supportive or transitional housing
  • Experience working with the adolescent/young adult population is preferred.

WORK ENVIRONMENT & PHYSICAL DEMANDS
  • Regular and consistent interaction with residents, Youth Counselors, and other program staff.
  • Will be required to travel between housing program sites, attend meetings and provide support across multiple locations.
  • Exposure to individuals experiencing crisis or behavioral health challenges.
  • Requires prolonged periods of sitting, standing, walking and moving throughout residential and office environments.
  • Requires the ability to use a computer, telephone, mobile device and other standard office equipment for extended periods.
  • May occasionally be required to lift, carry or move supplies, program materials, food and other items weighing up to approximately 25 pounds.
  • Requires the ability to navigate stairs and move throughout resident facilities as needed.
  • Evening, weekend, holiday or schedule flexibility may be required based on program needs.
