Base pay range

$50,000.00/yr - $55,000.00/yr

Position Summary:

We are searching for a highly motivated, independent, and creative case manager who is committed to making a difference in the lives of young people and empowering them to make positive life choices. The ideal candidate will have a passion for working with a population that consists primarily of youth (ages 13-21). The Case Manager will provide adolescents with services and referrals that will help them become more independent, assure emotional needs are met, expand their educational background, and help improve family communication and involvement.

Key Duties:

  • Support CAPP/Club TIA/AHWC with new patient/participant intake.
  • Coordinate resources, including reproductive health, behavior health/substance abuse treatment.
  • Develop with assigned youth, a strength-based service plan addressing the needs for treatment, training, medical, educational, vocational, social, and living skill development.
  • Assists youth and / or the youth's family in obtaining and maintaining basic life needs and skills.
  • Assist youth and their families to successfully utilize resources and services.
  • Responsible for overseeing the continuity of care of youth assigned.

Minimum Qualifications:

  • Bachelor's Degree in Social Work, Psychology, Human Services, or related field of study required.
  • 3-5 years of social worker, case management, or care coordination experience is required. Previous exposure to working in a community based organization, school setting, or youth programming is strongly preferred.
  • Proficiency with Microsoft Office.
  • Commitment to excellence and to providing quality service to patients.

Starting Compensation:

Minimum $50,000 per year; Maximum -$55,000 per year.

Work Environment Type, Location, and Schedule:

Onsite, Bronx, NY, Monday to Friday 9:00 am to 5:00 pm.
