Job Title: Case Manager
Classification: Full-time, Non-Exempt
Reports to: Assistant Program Director and Program Director
Location: Rustin House, New York, NY
Status & Hours: Full-time position with a regular daytime schedule that may include 1–2 evening or late shifts. Lantern staff may be asked to adjust their work schedule depending on the operational needs of the site and the agency.

Where You Will Work: Rustin House is one of Lantern’s supportive housing communities in New York City, serving formerly homeless individuals who may experience mental illness, substance use disorders, legal and criminal justice involvement, and other chronic medical conditions.

About the Role: We are seeking an experienced and dedicated Case Manager to join our team at Rustin House. The Case Manager will assist residents in stabilizing their lives, maintaining housing stability, and achieving their physical, emotional, vocational, and social goals. This role involves assessing client needs, linking clients to services, developing individualized plans toward stabilization and re-housing, monitoring progress, and maintaining accurate documentation.

Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Provide a range of services including benefits and entitlements assistance, case management, group programming, individual counseling and support, advocacy, resources, and referrals for residents.
  • Utilize a harm reduction approach to support a diverse caseload of formerly homeless individuals around issues of mental health and substance use.
  • Actively engage residents through regular apartment visits, community outings, and escorts to appointments as needed.
  • Develop safety plans for residents and work with the team to de-escalate clients and collaboratively manage crises as they arise.
  • Maintain current and accurate documentation for all cases across all required reporting agencies, including progress notes, case contacts, and case summaries.
  • Participate in staff and clinical meetings and individual supervision to ensure best practices in the performance of role responsibilities.
  • Perform other duties as assigned.

Required Education and Experience:

  • High school diploma, HSE, or GED required. Bachelor's degree from an accredited institution preferred.
  • Minimum of two (2) years of social services experience required.
  • Possession of a valid Food Handler's License, or the ability to obtain one within six (6) months of hire, is required.
  • Highly organized with the ability to manage a diverse caseload of clients with varying needs.
  • Able to work well under pressure, including in crisis situations, and flexible with work schedule when necessary.
  • Experience with ETO or other electronic case management systems.
  • Experience working with individuals impacted by homelessness, mental illness, substance use, and/or low-income backgrounds.

Work Environment: This position operates within a supportive housing site in New York, NY, and works with a population that may experience mental illness, substance use, disability, HIV/AIDS, and/or other chronic health conditions.

Physical Demands: While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. They must also use their hands to type, handle, or feel. The employee is frequently required to talk or hear and must occasionally lift and/or move up to 10 pounds. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this role.
