Job Overview:

The Housing Case Manager (HCM) is responsible for case management and supportive care coordination to asylee families.

Essential Duties and Responsibilities:

1. Primary: Provide case management services (psychosocial assessments, developing treatment plans, assisting with vital documents, benefits applications, housing placement, etc.) to approximately thirty (30) families.
2. Secondary: Maintain client records according to funder and regulatory standards.
3. Additional responsibilities include:
   • Referrals and follow-ups for primary care, substance use, nutrition, mental health, and more.
   • Coordinate with internal departments and external services to support families.
   • Assist clients with moving into apartments and transitioning from homelessness to stable housing.
   • Develop individualized goals collaboratively with clients.
   • Provide feedback on goal participation, discuss financial management, and accompany clients to appointments as needed.
   • Participate in supervision and complete reports timely.
   • Deliver excellent customer service and participate in advocacy efforts.
   • Maintain confidentiality, understand harm-reduction practices, and work in the office 100% of the time with community travel as required.
   • Work one weekend day each month.

Qualifications:

Bachelor’s degree in social work, psychology, counseling, or related field preferred, or three years of relevant social service experience.

Additional Requirements:

• Proficiency in Microsoft Outlook, Office, and CARES system.
• Dependability, flexibility, high confidentiality, and good communication skills.
• Understanding of harm-reduction practices.
• Ability to travel throughout NYC using public transportation.
