Role Description

This is a full-time, on-site role for a Case Manager, located in the New York City Metropolitan Area. The Case Manager will be responsible for assessing clients' needs, developing and implementing individualized service plans, providing counseling and support, and coordinating with community resources. The Case Manager will also monitor and evaluate clients' progress, maintain accurate records, and advocate for clients' rights and needs. Additionally, the Case Manager will collaborate with other team members to ensure comprehensive care and support for clients.
