Position Overview
The "Case Manager," formerly known as the Qualified Intellectual Disabilities Professional (QIDP) is under the supervision of the Network Director or their designee, performs highly responsible duties related to the development and implementation of plans and strategies for people receiving supports and services. The QIDP maintains, reviews and evaluates pertinent program data or information relative to each assigned person's growth and development. In addition, QIDPs maintain a leadership role by explaining and interpreting agency policy and service principles with special emphasis given to community integration, empowerment, normalization, etc.

Position Overview

The QIDP maintains, reviews and evaluates pertinent program data or information relative to each assigned person's growth and development. In addition, QIDPs maintain a leadership role by explaining and interpreting agency policy and service principles with special emphasis given to community integration, empowerment, normalization, etc.
Essential Functions
- Guide and lead the Community Support Team in providing individualized supports and services for people as they work towards accomplishment of their desired and chosen outcomes and goals. Those that a QIDP is responsible for supporting will be discussed and assigned at the discretion of the Network Director. At any given time, the QIDP may need to provide supports to additional people, on a temporary basis, based on the needs of the department.
- Complete all service documentation as required by the Department of Human Services (DHS) and/or Trinity within designated timelines.
- Conduct Personal Outcome Measure interviews annually, or more often as needed and/or requested.
- Provide additional support and assistance by working two late nights (11-7:30pm), within assigned residential sites, per week.
- Ensure the preferences and goals of people supported are reflected in various person-centered documents/plans and are updated as required or as needed/ requested.
- Provide recommendations to the Network Director regarding hiring, terminating, promoting, and evaluating Independent Living Coaches.
- Convene and facilitate Community Support Team Meetings annually or more often as needed/required to celebrate accomplishments as well as develop, discuss and/or update Flourish Plans and other required documentation as assigned.
- Convene and facilitate Community Support Team Meetings as needed/requested to collaborate and discuss concerns and decisions regarding the status and care of people supported.
- Make suggestions and recommendations to the Community Support Team regarding any additional services and supports that may be beneficial to the person's health and well-being.
- Facilitate and coordinate additional supports that may be needed (i.e. additional funding considerations, Support Service Team referrals, consultants, etc...)
- Assist with the coordination of benefits and financial accounts (i.e. social security, Medicaid/Medicare, bank accounts, etc...)
- Together with the Network Director and/or Associate Director and Team Leaders, ensure that services and supports are individualized and implemented appropriately and as needed in order to continue to meet the needs of all people served.
- Provide temporary oversight and supervision of community residential sites, which includes on-call responsibilities, when necessary.
- Provide personal support and assistance in gaining access to vocational services, recreation and leisure activities, social experiences, necessary consultant services, etc.
- Promote natural support networks to facilitate the development and maintenance of relationships
- Serve as an advocate of people's rights and services. When necessary, participate in Trinity's Human Rights Committee meetings. This should include involving the person in the process to the extent that they desire and/or are able.
- Complete Service and Support Agreements with people and their Community Support Team, addressing all concerns and expectations.
- Ensure the coordination and implementation of the overall health and well-being of people supported in accordance with DHS regulations, Trinity's policies and practices, physician recommendations, and the person's preferences, etc... This may include assisting with hospital and/or nursing home coordination or following medical consultations, as needed.
- Understand and apply related regulatory standards, including but not limited to, the Illinois Department of Human Services rules and regulations.
- Assist in providing on-the-job trainings and other onboarding trainings to Independent Living Coaches and other staff as needed in regards to person centered plans and strategies, outcomes, outcome tracking, and data for and related to the implementation of behavior support plans.
- Provide coaching and mentoring to Independent Living Coaches and other staff as needed
- Monitor and review progress towards identified outcomes on a monthly basis for each person supported, completing required documentation timely, and making adjustments in service provision as needed (under the guidance of the Network Director).
- Attend and participate in programmatic and organizational meetings and trainings as assigned.
- Serve as the champion in assisting people supported to progress towards and achieve their priority outcomes by providing direct support in the home and in the community and celebrating such achievements (i.e. TADAs)
- Make suggestions for program or organizational improvements, shares ideas with Trinity colleagues.
- Seek opportunities for personal development.
- Maintain registered QIDP status by completing 12 hours of continuing education annually (six of the twelve hours must be sponsored by non-Trinity sources)
- Complete initial medication authorization training (residential QIDPs) and maintain authorized status by attending required trainings.
- Complete other duties as assigned based on the needs of the organization.

- Able to take initiative and remain self-motivated in order to structure time effectively and ensure continued productivity
Physical Demands
- Be able to lift over 40 pounds from floor to waist unassisted
- Be able to push and pull 75 pounds
- Be able to complete deep knee squats
- Be able to push wheelchairs up to 200 pounds
- Remain in a stationary position, often standing or sitting for prolonged periods
- Operate motor vehicles
- While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach, stoop kneel to install computer equipment
- Physical activities that could be used include: driving, balancing, pulling, pushing, standing, climbing, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, and repetitive motion.
Qualifications
- Requires a Bachelor's Degree in a Human Services field, as determined by the Illinois Department of Human Services and have one-year experience working directly with individuals with intellectual/development disabilities.
- As a prerequisite, the position requires that a QIDP meet the state and federal job and educational requirements to perform work as a Qualified Intellectual Disabilities Professional. Once the person has met the requirements, they must be approved by Illinois Department of Human Services to be added to the state QIDP database.
- Requires demonstrated ability to train, coordinate and guide support staff from a variety of backgrounds as well as work with people supported of varying backgrounds and abilities.
