Job summary

Legacy Treatment Services is seeking a dedicated Case Manager in New Jersey to ensure medical needs of youth are met and to train Foster Parents on medication policies. The role collaborates with prescribers and conducts monthly medication monitoring for those on psychotropic medications.

The position includes on-call crisis support, coordination with outside agencies, and maintenance of electronic medical records. A bachelor’s degree in a mental health field or social work is required.

Qualifications

  • Bachelor’s degree required in a mental health-related field or social work.
  • Case management services required.

Responsibilities

  • Ensure medical needs are met for youth.
  • Train Foster Parents on medication policies and procedures including TFC Medication Manual.
  • Provide retraining on medical/medication needs to Foster Parents as needed.
  • Work with prescriber to communicate psychiatric or behavioral concerns.
  • Schedule monthly medication monitoring for new and ongoing psychotropic medications.
  • Maintain communication with outside agencies and other involved parties.
  • Attend Treatment Team meetings to provide medical information and therapeutic input.
  • Provide therapeutic support to clinician.
  • Serve as on-call staff for crisis after hours.
  • Assist with SCIP/Crisis Unit emergencies.
  • Assist with consumers at home or in office when behavioral issues arise.
  • Maintain electronic medical records.
  • Complete required annual trainings.
  • Resolve pharmacy issues.
  • Assist with intake process as needed.
  • Provide transportation for medical appointments and other needs.
  • Educate consumers regarding proper hygiene.
  • Complete incident reports.
  • Medication maintenance at intake and discharge.
  • Medication disposal as needed.

Education

Bachelor’s Degree in mental health field, social work, or related field

Job description

The Case Manager will:
  • Ensure Medical Needs are met and maintained for youth
  • Ensure Foster Parents are trained on Medication Policies and Procedures including TFC Medication Manual
  • Provide additional re-trainings as needed regarding the medical/ medication needs to Foster Parents
  • Work closely with the prescriber to communicate any psychiatric or behavioral concerns regarding the consumers in care.
  • Schedule monthly medication monitoring for incoming consumers as well as ongoing for consumers who are prescribed psychotropic medications
  • Maintain communication with outside agencies and/or other parties involved
  • Attend Treatment Team meetings to provide medical information/support as well as add therapeutic input
  • Provide therapeutic support to clinician
  • Serve as on-call staff to provide therapeutic support to foster parents with crisis after hours
  • Assist with consumers at SCIP/Crisis Unit on an emergency basis
  • Assist with consumers in home or in office when behavioral issues arise
  • Maintain electronic medical records
  • Complete required annual trainings
  • Resolve pharmacy issues
  • Assist with intake process as needed

Hours

Position is full time and includes the following:

  • 3.2 weeks of Paid Time Off during the first year

Education/Experience:

Bachelor’s Degree in mental health field, social work, or related field. Case management services required.
