Job summary

Believe In Me Empowerment in New Haven, CT is seeking a dedicated Case Manager to help individuals and families access essential resources and support. You will assess client needs, design personalized plans, and connect clients with housing, mental health, and employment services to promote independence.

Your role focuses on advocacy, documentation, and collaboration with community partners, ensuring culturally competent, trauma-informed care for diverse clients in the New Haven area.

Responsibilities

• Ability to manage a caseload effectively while maintaining attention to detail
• Empathetic, culturally competent, and trauma-informed approach to client care
• Proficiency in case management software and Microsoft Office Suite
