Job summary

Fresh Coast Alliance in Muskegon, Michigan, is seeking a dedicated Case Manager to provide case management services and support to participants in the sober living housing program.

You will screen applicants, orient participants, conduct assessments, meet weekly to review progress, and maintain accurate records while coordinating with the treatment team to ensure individualized plans and successful transitions.

Qualifications

• Associate's degree in Human Services or related field required; Bachelor's preferred.
• Two years of sustained sobriety is mandatory; prior incarceration experience strongly desired.
• Two years of case management experience in clinical/social service settings preferred.
• Peer Recovery Coach certification is advantageous.
• Knowledge of community resources for individuals with a history of substance use/incarceration is required.
• Understanding of the recovery process and needs of formerly incarcerated individuals is essential.
• A valid driver's license is required.

Responsibilities

• Participate in screening and selection of applicants seeking sober living.
• Provide orientation to incoming housing participants outlining program rules and expectations.
• Conduct assessments to determine goals, barriers, resource needs and refer to community resources as appropriate.
• Meet with participants on a weekly basis to review and discuss progress in the program.
• Maintain accurate and updated records of all interactions with clients and progress toward program goals.
• Provide ongoing monitoring and reporting of progress; communicate in weekly interdisciplinary meetings.
• Attend all meetings and trainings as required.
• Verify clients’ attendance at groups, treatment sessions or other services required by the plan.
• Enforce program rules and provide disciplinary action for rule violations.
• Intervene in crisis situations.
• Coordinate with the treatment team, recovery coach, Director of Housing and others involved in planning.
• Work with participants to develop exit plans and establish goals.
• Prepare discharge plans; coordinate discharge and post-discharge resources.
• Other duties as assigned.

Monday - Friday 8am - 5pm. Nights and weekends as needed.

Job Summary

The Case Manager provides case management services and support to participants in the sober living housing program at Fresh Coast Alliance.

• Provide orientation to incoming housing participants to outline program rules and expectations.
• Conduct assessments with participants to determine goals, barriers, resource needs and other factors related to case planning, referring them to community resources as appropriate.
• Meet with participants on a weekly basis to review and discuss progress in the program.
• Maintain accurate and updated records of all interactions with clients and any other contacts made on their behalf. Document participants progress toward program goals.
• Provide on-going monitoring and reporting of participant’s progress. Communicate clients’ progress by conducting weekly interdisciplinary meetings and evaluations;
• Attends all meetings and trainings as required
• Verify clients’ attendance at any groups, treatment sessions or any other services that are required as part of the participants program plan.

Minimum Qualifications

• An Associate's degree in Human Services, Social Work, Criminal Justice, or a related field is required; a Bachelor's degree is preferred.
• Demonstrated experience with long-term sobriety or prior incarceration is strongly desired; a minimum of two years of sustained sobriety is mandatory.
• A minimum of two years of case management experience within a clinical/social service environment or direct experience working with clients possessing a history of substance use and/or incarceration is preferred.
• Peer Recovery Coach certification is advantageous.
• Comprehensive knowledge of community resources and service programs for individuals with a history of substance use and/or incarceration is required.
• A thorough understanding of the substance use recovery process and the distinct needs of formerly incarcerated individuals is essential.
• A valid driver's license is a prerequisite.
