Multi-site Case Manager

RN or LPN Preferred

CommuniCare Health Services is currently seeking a healthcare professional with case management experience for a full-time Multi-site Case Manager position across multiple facilities in West Virginia. This role is based out of our Morgantown, WV office.

Job Responsibilities

The Multi-site Case Manager's duties include:

  • Case management
  • Precerts, authorizations, and concurrent reviews
  • Management of utilization, therapy minutes, insurance requirements, etc.
  • Coordination and communication with insurance companies
  • Collaboration with the IDT team
