Job summary

Community Care is seeking a Case Manager for a full-time role in Dane County, WI. The position supports elderly individuals and adults with disabilities, coordinating care plans and services to promote independence in the community.

Eligible candidates will have a Bachelor’s Degree in Human Services, and at least 3 years’ experience with seniors or disabilities; a long-term care functional screener certification is required or eligible. Mon-Fri, 8:00 AM-4:30 PM, with local travel.

Qualifications

• Bachelor’s Degree in a Human Services or closely related field required.
• Minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human services degree.

Responsibilities

• Manages an assigned caseload of program members in collaboration with an RN Case Manager.
• Completes comprehensive member assessments, member centered care plans, & functional screens.
• Provides ongoing care coordination and quality assurance monitoring of services.
• Maintains required documentation in adherence to established standards.

This full-time opportunity offers a weekday schedule and a $8,000 SIGN-ON BONUS. Position located in Dane Co.

Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Case Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager Job Requirements:

• Education: Bachelor’s Degree in a Human Services or closely related a rea required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred .
• Certification: Eligibility for long-term care functional screener certification is required
• Travel : Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Case Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
