Job summary

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a compassionate CCS Case Manager to join the Comprehensive Community Services team in Dane County. This full-time, 40-hour-per-week position coordinates services, provides psychoeducation, and helps individuals achieve greater independence and well-being in their homes, schools, and communities.

Responsibilities

• Develop individualized service plans in partnership with clients, families, support systems, and providers.
• Coordinate and authorize services as appropriate.
• Facilitate recovery-focused and family-centered team meetings.
• Connect clients to community resources and support services.
• Maintain accurate client records and documentation.
• Complete required reports and service documentation on time.
• Collaborate with treatment teams, community providers, schools, healthcare professionals, and natural supports.
• Participate in supervision, training, and staff development opportunities.

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a compassionate and dedicated CCS Case Manager to join our Comprehensive Community Services (CCS) team in Dane County. This community-based role supports children, adolescents, adults, and families experiencing mental health challenges through a person-centered, recovery-focused approach.

What You’ll Do
