Job summary

The Lutheran Social Services of Wisconsin & Upper Michigan seeks a Service Facilitator for its Comprehensive Community Services (CCS) program in Trempealeau County, WI. This full-time, 40-hour/week position focuses on assessing needs, planning services, and coordinating supports to promote independence and well-being.

Lutheran Social Services (LSS) is a private, non-profit, human service agency providing a variety of services throughout Wisconsin and Upper Michigan. Comprehensive Community Services (CCS) Program

Location: Trempealeau County, WI

Schedule: Full-Time, 40 hours per week

LSS is seeking a compassionate and organized Service Facilitator to partner with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate services that promote independence, well-being, and growth.

Compensation based on education and/or credentials:

Bachelors: $24.20/hr

Master's Degree: $27.20/hr

Master's w/therapy licensure: $28/20/hr

What You'll Do

What We're Looking For

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or a related field.
• Experience working with individuals, families, or vulnerable populations preferred.
• Strong communication, organization, and relationship‑building skills.
• Ability to manage documentation, coordinate multiple services, and work independently.
• Valid driver's license, reliable transportation, and ability to travel within the community.
