UW Health Rehabilitation Hospital is a free-standing, 50-bed acute inpatient rehabilitation facility located on the east side of Madison, Wisconsin. Opened in September 2015, the hospital offers specialized programs for individuals who have experienced stroke, brain and spinal-cord injuries, amputations, complex orthopedic injuries, and other conditions requiring inpatient rehabilitative services. Our goal is to help patients achieve their highest level of recovery and return home as soon as possible. UW Health Rehabilitation Hospital is a partnership between UW Health, Unity Point Meriter, and Kindred Healthcare.

Role Description

This is a full-time on-site role for a Case Manager. The Case Manager will be responsible for evaluating patients' needs, developing individualized care plans, coordinating with healthcare providers, and ensuring that patients have access to necessary services. The role also involves monitoring patients' progress, providing support to patients and their families, and maintaining accurate and up-to-date records. This position is located in Madison, WI.

Qualifications

• Experience in developing and managing individualized care plans and coordinating patient care
• Strong communication, interpersonal, and organizational skills
• Ability to work collaboratively with healthcare providers, patients, and families
• Experience with accurate record keeping and documentation
• Knowledge of rehabilitation processes and healthcare services
• Relevant certifications and licensures such as RN, LCSW, or similar are a plus
• Bachelor's degree in Nursing, Social Work, or a related field is preferred
