The Organization

SSG-Project 180 turns lives around through innovative, wraparound services that keep people out of institutions and in their communities. Project 180 is a leader in the forensic treatment space, offering high-quality, specialized treatment programs that serve thousands of justice-involved individuals. Our programs span criminal justice intercepts and include pre-arrest interventions, court-based diversions, jail in-reach, and intensive post-release services. Project 180 operates in partnership with County agencies including the Department of Mental Health (DMH), the Department of Health Services (DHS), and the Justice, Care and Opportunities Department (JCOD).

The Program

The Rapid Diversion Program (RDP) is a County-operated program that diverts criminal defendants out of the criminal legal system at the early stages of the court process. The program provides in-court mental health evaluation, linkage to treatment services, and case management support to participants, whose criminal case is dismissed upon successful program completion.

The Position

Under the supervision of the Program Supervisor, the RDP Case Manager is responsible for providing wraparound, field-based case management and rehabilitative services to a caseload of approximately 20 diversion clients. Success in this role depends on actively engaging clients, supporting their recovery goals, and utilizing community resources effectively. Job title (I-III) will be assigned based on relevant experience.

Essential Functions
  1. Provide wraparound, field-based services to support recovery goals
    • Engage clients through rapport building, consistent contact, and motivational interviewing. Meet clients where they are at, including outreach at service locations.
    • Deliver services aligned with recovery-oriented care principles, including hope, empowerment, and trauma-informed practices.
    • Develop knowledge of community resources, assess needs, and facilitate linkage and problem-solving to overcome barriers. Accompany clients to appointments when appropriate.
    • Teach prosocial and recovery skills such as communication, problem-solving, and anger management.
    • Engage with family, court, and other collaterals as appropriate. Provide mental health recovery education and advocacy.
    • Conduct custody pickups from LA county jails.
  2. Support clients in adhering to court requirements
    • Help clients understand the court process and their responsibilities in diversion.
    • Assist with court attendance, transportation, and accompaniment.
    • Obtain and submit progress reports from providers within deadlines.
    • Maintain collaboration with RDP court staff to ensure quality services.
  3. Documentation and Reporting
    • Keep detailed case notes in the electronic health record.
    • Participate in data collection activities as required.
  4. Fulfill baseline staff expectations
    • Attend work regularly, including holidays or weekends if needed.
    • Uphold agency mission, policies, and procedures.
    • Perform additional duties as assigned.
    • Maintain active licenses or registrations (e.g., BBS, CCAPP).

Minimum Qualifications
  • Experience working with diverse populations.
  • Valid CA driver’s license, proof of insurance, and own vehicle.
  • No current legal supervision (probation or parole).
  • Background check and employment verification.
  • Basic computer skills.
  • Case Manager I: Less than 2 years of experience.
  • Case Manager II: 2+ years of experience.
  • Case Manager III: 3+ years of experience.
Preferred Qualifications
  • Previous case management experience.
  • Lived experience with homelessness, mental health/substance use recovery, or the criminal justice system.
  • Experience with forensic or dual-diagnosed populations.
  • Spanish language skills.

Environmental Conditions

Work is performed in both office and field settings, including courthouses.

Physical Requirements

Must be able to move around the worksite, including walking, standing, climbing stairs, and driving. Capable of lifting up to 20 lbs, listening, and speaking to fulfill job duties.

Mental Requirements

Must perform complex tasks, handle distractions, interact professionally in high-stress situations, create and follow plans, and meet deadlines.
