Job summary

Merakey is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team in the Huntingdon, PA area. The role focuses on assessments, planning, and transferring recovery skills for adults and youth with mental health diagnoses or autism spectrum disorders, using a person-centered approach.

The team collaborates across settings to deliver intensive therapeutic services and supports, with opportunities for growth and a strong commitment to compassionate care.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Criminal Justice, or similar human services required.
• Experience in the behavioral health field working with individuals with mental health/developmental disabilities required.
• Valid PA driver’s license with a clean driving record and reliable transportation required.
• Possesses or is able to obtain all necessary clearances and background checks to work with designated consumers.
• This is a community based role, so travel is required.

Responsibilities

• Assess, plan, and transfer recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum.
• Provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc.
• Collaborate with other members of the treatment team to implement and transfer skills from the recovery treatment and support plan.

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
