As a Case Manager, you will work collaboratively with participants, families, clinical staff, medical providers, and community partners to coordinate services and connect individuals with the resources they need to achieve their goals. This role involves community-based case management, advocacy, service coordination, documentation, and participation as part of a multidisciplinary treatment team. You will support a holistic, strengths-based approach to services while promoting participant choice and self-determination.
