  • Within 1 year of obtaining certification in Hawaii as a Substance Abuse Counselor or higher education in related field.
  • CPR and First Aid Certification.

Responsibilities

  • Assesses clients to determine eligibility for case management services.
  • Assists clients in obtaining services promoting self-efficacy and referrals.
  • Collaborates with service providers to coordinate CM activities per client needs and funder requirements.
  • Provides clients with lists of appropriate providers and helps schedule appointments.
  • Ensures CM is available in locations that meet client needs and includes CM in treatment plans.
  • Maintains client records with referral details, contact dates, communications, and outcomes.

Position Summary

Under the supervision of the Case Manager Supervisor, the Case Manager carries out casemanagementandservicescoordination(CMS)withclients,inaccordancewiththe Hina Mauka policies and procedures. CMS is designed to provide clients with case management and services coordination activities that offer goal-orientated and individualized support focusing on improved self-efficacy.

Case management and services coordination are based upon the client’s individual needs and may include assistance related to health care, housing, social services, employment and recreational activities. Hina Mauka will comply with all CARF regulations in order to assure quality control.The Case Manager protects the confidentiality of clients and client records in accordance with all applicable Federal regulations.

Essential Functions
  1. Assessesifclients meetcriteriafor CM.
  2. Assistsclientsinobtainingservicesthatpromoteself-efficacy.Thereferralprocess may occur at any point during the treatment process.
  3. CollaborateswithpersonsservedintheprovisionofCMserviceswithclientsbeing served. Elements provided are dependent on the needs of persons being served and/or funder/regulatory requirements.
  4. Providesclientswithlistsofappropriateserviceproviders,andassiststhemin scheduling appointments with relevant providers.
  5. MakeseffortstoprovideCMinlocationsthatmeettheneedsofthepersonsserved.
  6. IncludesoutsideservicesandbenefitsgainedfromCMaspartoftheclients' treatment plans.
  7. EnsuresthatclientssignaConsenttoReleaseInformationformpriorto contacting any outside agency/provider on behalf of clients.

  1. MaintainsclientrecordsinaccordancewithHinaMaukapoliciesandprocedures and in compliance with all state and federal laws, licensing and accreditation standards and contract stipulations.
  2. Completesfiscal,statisticalreportingandqualityimprovementtasksasrequired.
  3. Be available 24/7 to clients during emergencies as required by AMHD contract.
Other Functions

Qualification Requirements Educational/Experience and Training:
  1. CertificationasaSubstanceAbuseCounselor(CSAC)oranAdvanceDegreein Social work, Psychology or Behavioral Science preferred.
  2. Within 1 year of obtaining certificationin the State of Hawaii as a Substance AbuseCounselorand/orreceivinghighereducationinthefieldofSocialWork, Psychology or behavioral science.
  3. CPR and First Aid Certification.

  4. Abilityto work independentlyandcooperativelywith others.
  5. Demonstrate the Hina Mauka values in interactions with persons served, their supportservicesandothersinthecommunitywhoareengagedwithHinaMauka.
Physical/MentalDemands

Light physical effort (lift/carry up to 10 pounds); occasionally lifts supplies/equipment. Occasional standing/walking/driving. May be exposed to infectious and contagious diseases. May perform emergency care.
