Job summary

North American Family Institute (NAFI) is seeking a full-time Case Manager to support adolescent youth and families involved with the Florida Department of Juvenile Justice. Based in Jacksonville, you will provide direct case management, assist a Family Therapist, and visit residential programs.

You will coordinate transports, maintain records in electronic health records, and collaborate with a multidisciplinary team to deliver services to youth and families in the community and residential

Qualifications

• Bachelor’s degree in a related human services field.
• Experience working with juvenile justice or at‑risk youth.
• Strong teamwork and communication skills.
• Valid Florida driver’s license, clean driving record, and reliable transportation.
• Flexible schedule, including some evening hours.
• Comfortable with computers, data entry, and electronic health records.
• Background screening and proof of highest education level required prior to hire.

This position is based out of our Jacksonville office and focuses on providing case management services, assisting a Family Therapist, and conducting visits to residential programs.

Key Responsibilities

• Provide direct case management services to youth and families

• Transport clients as needed
• Maintain accurate records and documentation in electronic health record systems

Requirements

Compensation

Compensation for this position is $18.50 per hour ($38,480 a year)
