Position Overview

The Case Manager is a vital member of the hospital or specialty-based patient-centered care team. Responsible for collaborative practices including identification, assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet patients' healthcare needs. The primary goals are to promote patient safety, quality care, and cost-effective outcomes. The case manager provides effective discharge planning services to ensure safe transitions to subsequent care levels and manages needs of patients experiencing critical events or diagnoses to optimize health outcomes across the care continuum.

Work Schedule

36-hour days: 9:00 AM to 9:30 PM.

Qualifications

• Bachelor's degree in Nursing or Master's in Social Work.
• Minimum of 3 years of clinical experience.
• Excellent communication, documentation, customer service, and interpersonal skills.
• Strong problem-solving, analytical, decision-making, and computer skills.
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care preferred.
• Knowledge of clinical guidelines, Medicare/Medicaid regulations, case management principles, and social determinants of health.
• Ability to facilitate community resources for diverse populations.
• Strong organizational and advocacy skills.

Licensing and Certifications

• Registered Nurse (RN) or Licensed Social Worker (LMSW) with valid Michigan license.
• Preferred certifications: CCM or ACM.
