JOB SUMMARY: The Case Manager will providecomprehensive case management services to clients and theirfamilies/significant others to ensure they have access tocomprehensive, community-based services, and that all requirementsfrom referral sources are met. This position requires you todrive or be open to learing how to drive an RV This position mayexpose you to heat, cold, rain or inclement weather
KEY RESPONSIBILITIES:
- Focuses on engaging clients from the time of the referral throughthe entire course of the client’s treatment services.
- Assists clients in connecting with and gaining eligibility forentitlement programs, including public and/or private insurance programs.
- Conducts standardized screening for health-related social needsand offers resource and referral services specifically tailored tothat client.
- Provides linkage and navigation assistance for clients and theirfamilies/significant others so they can access all relevantcommunity services.
- Helps clients and their families identify and access other Villageclinical services and/or other community resources.
- Maintains and updates service data to ensure accurate reportingwhen required.
- Develops and maintains collaborative relationships with othercommunity agencies, organizations, and referral sources.
- Complies with all agency policies and procedures, HIPAAregulations, Joint Commission standards and funding sourcesrequirements including complete, timely, and accurate documentationof all relevant client information as required.
- Demonstrates cultural competence necessary to serve diverse client population.
- Participates in a collaborative team approach and engages inongoing quality improvement efforts to enhance the functioning ofthe program.
- Contributes to agency visibility and development of communitylinkages by representing The Village in community groups orcommittees, when directed by Supervisor.
- Must be able to drive an RV
- Performs other duties and responsibilities as assigned by management.
Experience and Skills
EDUCATION
- Bachelor’s degree in Social Work or Human Services required.
EXPERIENCE
- At least 3 years of direct experience in SUD treatment, withfamiliarity with CM protocols and evidence-based practices.
- Must be culturally competent to serve diverse client population;bi-lingual (English/Spanish) strongly preferred.
- Excellent oral and written skills and ability to communicatecomfortably with children, parents, and both primary care andbehavioral health professionals.
- Organized and able to work independently with minimal supervision.Ability to keep organized records of monetary reward systems.
- Proficient with word processing and spreadsheet applications.Possess solid general computer skills, including ability to work ina Windows environment.
