Job summary

Path Behavioral Healthcare seeks a dedicated Case Manager in Greenville, OH to coordinate care and support clients’ behavioral health needs. You will assess, plan, implement, coordinate, monitor, and evaluate services to promote health, recovery, and cost-effective outcomes while ensuring patient safety.

You will work with therapists, family members, and community resources to facilitate access to housing, employment, education, and health services, while maintaining documentation and HIPAA

Qualifications

  • Education: High School Diploma or GED; Bachelor’s degree preferred.
  • Experience: Minimum of 3 years of case management experience.
  • Communication: Excellent verbal and written communication skills.
  • Interpersonal: Ability to interact professionally with clients, families, and providers.
  • Technical: Competency in EHR programs and telemedicine tools.
  • Background: Must pass all federal and state background checks, including MVR.

Responsibilities

  • Act as liaison among therapist, care team, client, and family to coordinate care.
  • Maintain clear communication with family, referrals, and Path Behavioral Healthcare.
  • Attend court hearings as appropriate.
  • Ensure documentation and billing align with policy and EHR standards.
  • Monitor chart/documentation audits and Phase System of Care requirements.
  • Attend supervisory and team meetings; present case management reports.
  • Facilitate individual and group meetings as assigned.
  • Assist in quality assurance committees as assigned.
  • Develop daily living skills as identified by client or guardian.
  • Oversee implementation of the ISP and service plan.
  • Ensure client participation in plans, goals, status.
  • Identify and address gaps in service and monitor utilization.
  • Support communications with referral sources.
  • Coordinate crisis intervention and discharge planning.
  • Maintain organized practice and be ready to work.

TITLE: Case Manager, Behavioral Health RESPONSIBLE TO: Regional Leadership WORK AREA: Outpatient sites and Community

SUMMARY: The Case Managers primary role, under the direction of Regional Leadership and the Case Manager Supervisor, is to provide support and coordinating services based on client needs and treatment plans. The case manager will actively engage in a professional, collaborative process to support our clients' mental and physical health needs. As a Case Manager, you will assess, plan, implement, coordinate, monitor, advocate, and evaluate the options and services required to promote health, quality, and cost-effective outcomes for our clients, ensuring patient safety and adherence to quality care standards.

In this role, you will work closely with other care team members to plan, link, advocate, coordinate, and monitor client care. This collaboration will help clients access a wide range of resources, including but not limited to health services, financial assistance, housing, employment, education, and social services. The Case Manager is also responsible for successfully managing their caseload and communicating with the treatment team as it relates to Case Management across various disciplines.

DUTIES AND RESPONSIBILITIES
Day-To-Day Responsibilities
  • Actasaliaisonbetweenthetherapist,othercareteammembers,client,andclient'sfamily to coordinate the best care.
  • Maintainclearandongoingcommunicationbetweenthefamily,referralsource,andPath Behavioral Healthcare.
  • Attendcourthearingsasappropriate.
  • Ensuredocumentationandbillingadheretopolicy,PathBehavioralHealthcarestandardsof practice, and all applicable compliance standards within the EHR system.
  • Maintaincompliancewithchart/documentationauditcriteria,billingreviews,andPhase System of Care requirements.
  • Attendsupervisoryandteammeetings;presentaccuratecasemanagementreportsreflecting services provided and clients' responses to treatment.
  • Facilitateindividualandgroupmeetingsasassigned.
  • Serveonagency qualityassurance orothercommitteesasassigned.
  • Facilitatethefurtherdevelopmentofdailylivingskillsifidentifiedbytheclientand/or parent or guardian.Addressandresolveclient/guardianconcernsinatimelymanner.
  • Oversee the implementation of the individual plan of service, supporting the client's aspirations,goals,anddesiresforoptimizingindependence,promotingrecovery,and developing andmaintainingnatural support systems.
  • Ensureongoingclientparticipationindiscussionsoftheirplans,goals,andstatus.
  • Identifyandaddressgapsinservice,aswellasmonitorunderandover-utilizationof authorized services.
  • Identifyandassistinbusinessdevelopment/referral source communication and collaboration efforts.

QUALIFICATIONS

Education: Candidate must have a High School Diploma or GED. Bachelor’s degree is preferred.

Experience: Candidates must have a minimum of 3 years of case management experience.

Physical Effort: This position requires the ability to work under stressful conditions and to work irregular hours.

Communication Skills: Candidates must possess excellent verbal and written communication skills in order to communicate professionally via telephone and in writing. Strong grammar and writing skills are crucial for clear and concise communication with clients, colleagues, and other healthcare professionals.

Interpersonal Skills: The successful candidate must demonstrate the ability to interact and assist in a friendly, compassionate, and professional manner with colleagues, providers, clients, and their families, as they represent both themselves and Path Behavioral Healthcare.

Essential Technical/Motor Skills: Candidates must achieve competency in the Electronic Health Record (EHR) program, telemedicine services, and any additional software programs utilized by the company, ensuring efficient access and updating of client information. The role requires fine dexterity, adept handling, and proficient gripping abilities to perform various tasks.
