Job summary

McAlister Institute in San Diego is seeking a compassionate Case Manager to help clients access medical, educational, social, and vocational resources. The role focuses on coordinating SUD care and integrating with primary care for clients with chronic substance use disorders or co-occurring conditions, while assisting interactions with the criminal justice system.

The position is on-site and requires CCCAPP/CADTP certification, a minimum of two years in behavioral health, and a valid California

Qualifications

  • Certified or registered SUD Counselor required (CCCAPP, CADTP, or equivalent).
  • Minimum of two years in behavioral health, especially with SUD/co-occurring conditions and criminal justice involvement.
  • Strong knowledge of community resources and relationships with local agencies.
  • Bilingual proficiency in Spanish and English strongly preferred.
  • Valid California driver's license with a clean driving record.

Responsibilities

  • Uphold and promote the mission, philosophy, values, and direction of McAlister Institute.
  • Develop and manage individualized case management service plans to ensure clients have needed support and resources.
  • Perform comprehensive assessments and reassessments to tailor treatment plans.
  • Assist clients in applying for and maintaining health and public benefits (e.g., Medi-Cal, Healthy San Diego, Housing).
  • Provide recovery support through outreach, life skills workshops, socialization, and group activities.
  • Advocate for clients across systems to ensure access to physical and mental health care, housing and essential services.
  • Maintain accurate records and documentation adherent to DMC standards and agency policies.
  • Coordinate transitions in care across ASAM levels, including referrals and working with treatment agencies.
  • Develop and maintain relationships with community partners to access resources and services.
  • Facilitate transportation for clients to medical appointments and treatment sessions when necessary.

McAlister Institute is seeking a compassionate and dedicated Case Manager to join our team. This individual will report directly to the Program Manager and assist clients in accessing vital services such as medical, educational, social, and vocational resources. Focus will be on coordinating Substance Use Disorder (SUD) care, integrating with primary care for clients with chronic substance use disorders or co-occurring conditions, and supporting interactions with the criminal justice system as needed.

Key Responsibilities:

Requirements
  • Certified or Registered SUD Counselor (CCCAPP, CADTP, or equivalent required).
  • A minimum of two years of experience in behavioral health, particularly working with clients with substance use disorders, co-occurring conditions, and involvement in the criminal justice system.
  • Strong knowledge of community resources and established relationships with local agencies.
  • Proven ability to manage multiple tasks in a fast-paced environment, with excellent written and verbal communication skills.
  • Bilingual proficiency in Spanish and English strongly preferred.
  • Ability to maintain ethical, culturally competent, trauma-informed, and evidence-based service delivery.
  • Must possess a valid and unrestricted California Driver's License, with a good driving record, free of any major infractions.
  • On-site position.
  • Ability to pass all pre-employment screening, including references, education verification, a criminal background check with at least one year clear of probation or parole, a drug test, and a negative TB test.
