To keep families together by providing safe, permanent housing, and ongoing accessibility to evidence-based trauma informed support services to Child Protection and Permanency families throughout our region.
Provide support services and case management to families with a history of homelessness or housing instability.Assistparentsto makea successful transition to self-sufficient living andimprovedindividual and family stability.Provide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, advocacyskills.Co-facilitate groups for parents,childrenand families.Link families to both formal and informal community resources.Work collaboratively with your KFT peers, DCP&Pandcommunity service providers.Complete documentation consistent with program and agency requirements.As an integrated healthorganizationAcendavalues the use of evidence-based practice or EBP’s.
Serve as the contact point, advocate and informational resource for clients, care team,
family/caregivers and community resourcesregardingclient’sserviceplan
Partner with clients tomonitoradherence to theirserviceplan, evaluate effectiveness,
monitorprogressin a timely mannerandfacilitatechanges asrequired.
Facilitate client linkages, referrals and access toappropriate treatment, community
supports and medical or specialty providers according to theserviceplan
Maintain an active caseload ofapproximately 10-12families, providing in person in home therapeutic sessions to address treatment plan goals
Educate patient and family/caregiver(s) about relevant community resources
Cultivate andmaintainrelationships with external community providers andsupports.
Completes andsubmitsaccuratereports and chart documentation in a complete and
timelymanner according to program guidelines and established procedures.
Ensures quality and accuracy of written and verbal communications.
Support the Quality/Compliance/Risk function asrequired.
Performs other related duties as assigned.
Bachelor’s level practitioner withat least 2years of experiencein socialservice-relatedfield preferred.
Must have andmaintaina valid driver’s license with an acceptable driving record
Must be able to exercise independent thinking and good judgment under all circumstances.
Must be able to analyze work, set goals, developplansandutilizetime effectively and efficiently.
Bilingual rate: An additional $1.50 added to the base pay if Spanish proficient.
