Job summary

Acenda Integrated Health in Egg Harbor Township, NJ is seeking a Care Manager (Hourly). This role provides support services and case management to families with housing instability, helping them transition to self-sufficient living and improving stability for the whole family.

Responsibilities include coaching on life skills, linking to resources, maintaining a caseload of about 10–12 families, and delivering in-home therapeutic sessions while documenting progress and collaborating with the care

Qualifications

• Bachelor’s level practitioner with at least 2 years of experience in social service-related field preferred.
• Must have and maintain a valid driver’s license with an acceptable driving record.
• Able to exercise independent thinking and good judgment under all circumstances.

Responsibilities

• Provide contact and support to clients, care team, families and community resources.
• Monitor adherence to service plans and evaluate effectiveness; adjust as needed.
• Maintain an active caseload and provide in-home therapeutic sessions.
• Educate clients about community resources and document progress timely.

Program Overview:

To keep families together by providing safe, permanent housing, and ongoing accessibility to evidence-based trauma informed support services to Child Protection and Permanency families throughout our region.

PositionSummary:

Provide support services and case management to families with a history of homelessness or housing instability.Assistparentsto makea successful transition to self-sufficient living andimprovedindividual and family stability.Provide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, advocacyskills.Co-facilitate groups for parents,childrenand families.Link families to both formal and informal community resources.Work collaboratively with your KFT peers, DCP&Pandcommunity service providers.Complete documentation consistent with program and agency requirements.As an integrated healthorganizationAcendavalues the use of evidence-based practice or EBP’s.

Key Responsibilities:

• Serve as the contact point, advocate and informational resource for clients, care team,

• family/caregivers and community resourcesregardingclient’sserviceplan

• Partner with clients tomonitoradherence to theirserviceplan, evaluate effectiveness,

• monitorprogressin a timely mannerandfacilitatechanges asrequired.

• Facilitate client linkages, referrals and access toappropriate treatment, community

• supports and medical or specialty providers according to theserviceplan

• Maintain an active caseload ofapproximately 10-12families, providing in person in home therapeutic sessions to address treatment plan goals

• Educate patient and family/caregiver(s) about relevant community resources

• Cultivate andmaintainrelationships with external community providers andsupports.

• Completes andsubmitsaccuratereports and chart documentation in a complete and

• timelymanner according to program guidelines and established procedures.

• Ensures quality and accuracy of written and verbal communications.

• Support the Quality/Compliance/Risk function asrequired.

• Performs other related duties as assigned.

Requirements:

• Bachelor’s level practitioner withat least 2years of experiencein socialservice-relatedfield preferred.

• Must be able to analyze work, set goals, developplansandutilizetime effectively and efficiently.

Additional Information:

Base Pay: $19.23

Sign-on Bonus: $500

Schedule: Day shift

Bilingual rate: An additional $1.50 added to the base pay if Spanish proficient.
