Base pay starts at $20.19/hour plus a $1,000 sign-on bonus. Bilingual Spanish proficiency adds $1.50/hour. On-site work at Gloucester ICMS offices with travel to community sites is required.

This role works in collaboration and continuous partnership with clients, community services, and providers, and the clinician team to ensure the client's care plan is effectively implemented and adequately addresses the client’s needs. The role will connect clients to relevant community resources including substance abuse treatment, housing, employment, mental health services and primary care. On call as needed for client crisis needs.

As an integrated health organization, Acenda values the use of evidence-based practice or EBP's. Integrated Case Management Services (ICMS) engages individuals at the point of hospitalization and participates in the discharge planning process to assist with the transition from hospital to community. Case management provides targeted case management services and post-discharge follow-up to support the client with community integration. This is done through direct in-home contact and linkage to community resources. The client is linked to resources that support the ability to remain stable within the community. ICMS, identifies, assesses and develops plans to address the needs of the Seriously and Persistently Mentally Ill (SPMI) through targeted case management.
Case manager supports clients with face-to-face targeted case management, as designated through the ROH levels, needs as identified in contracts and regs, integration into the community, hospital discharge planning, and community linkages. Services shall include, but shall not be limited to: assessment, service planning, services linkage, ongoing monitoring, ongoing support, and advocacy. Provides weekly face to face contact with clients in the community by pre-scheduling sessions and utilizing treatment plan goals for session preparation. Provides linkages across needs, warm hands, and follow up for resources to ensure client stability; transport clients for identified goal needs and stabilization for successful linkage needs. Maintain all documentation as identified in state contract and program manual. Monitors treatment planning ongoing and discharge planning. Staff weekly performance benchmarks is between 20-25hrs of face to face, 50% of time is client facing. Face to face time also includes support to OCHA residents through weekly office hours, groups, and individual sessions on site. On-site presence at hospitals and consistent communication with other community agencies for referrals and pre-discharge planning. Participate in all program, agency, supervision meetings as required. Maintains yearly required case management trainings. Completes all required documentation: notes within 24hrs of contact, as well as treatment plans and assessments as documented in program manual to support timely billing and required timeframes. Adheres to a flexible schedule based on client and program needs. Demonstrates competent written and verbal communication with referral sources and external customers. Attentive and responsive to customer needs in a helpful, supportive and timely manner. Works collaboratively with coworkers and agency staff to effectively communicate and problem-solve. Meets obligations of the program (contractual, licensing, Medicaid, etc.). Follows program policies and procedures and encourages the same in others. Educates self on issues impacting the program population. Operates conscientiously in accordance with program budget (i.e., use of client engaging funds, office supplies). Performs other related duties as assigned.
While performing the duties of this job, the employee is occasionally required to remain in a stationary position, often standing or sitting for prolonged periods. Repeated motions that may include the wrists, hands and/or fingers The employee must occasionally move about to accomplish tasks, adjust or move objects up to 20 pounds. Must be able to communicate with others to exchange information.
