Job summary

Council on Aging of Volusia County in Daytona Beach, FL is seeking a compassionate Case Manager to help older adults live independently with dignity and respect. You will conduct home assessments, develop individualized care plans, coordinate services with community resources, monitor progress, and advocate for clients with thorough documentation.

A Bachelor’s degree is preferred; bilingual English/Spanish is a bonus.

Qualifications

• Bachelor's degree in Social Work, Human Services, Nursing, Gerontology, or a related field preferred; equivalent combination considered.
• Experience in case management, social services, healthcare, or working with older adults is a plus.
• Bilingual in English and Spanish is a plus and highly valued in serving our diverse community.

Responsibilities

What We're Looking For
