Job summary

Overview: As a Case Manager at Asurgent Health Addiction Treatment Center, you will play a crucial role in providing comprehensive support and assistance to individuals undergoing addiction treatment. You will work closely with clients, their families, and a multidisciplinary team to develop and implement personalized treatment plans aimed at promoting recovery and holistic wellness.

Responsibilities:

  • Assessment and Evaluation:

    • Conduct thorough assessments of clients' needs, strengths, and challenges related to addiction treatment.

    • Collaborate with clients and other team members to develop individualized treatment plans based on assessment findings.

  • Treatment Planning and Coordination:

    • Help develop comprehensive treatment plans that incorporate evidence-based interventions and address clients' physical, psychological, and social needs.

    • Coordinate with healthcare professionals, counselors, and other service providers to ensure seamless delivery of services and continuity of care.

  • Monitoring and Support:

    • Provide ongoing support and guidance to clients throughout the treatment process.

    • Monitor clients' progress, evaluate the effectiveness of interventions, and make adjustments to treatment plans as needed.

  • Advocacy and Resource Referral:

    • Serve as an advocate for clients, ensuring their needs are met and their rights are respected.

    • Identify and connect clients with community resources, support groups, and other services to facilitate their recovery journey.

  • Crisis Intervention:

    • Respond promptly and effectively to crises or emergencies involving clients.

    • Collaborate with team members to develop crisis management plans and ensure client safety.

  • Documentation and Reporting:

    • Maintain accurate and up-to-date documentation of client assessments, treatment plans, progress notes, and other relevant information.

    • Generate reports as required by regulatory agencies and funding sources.

Qualifications:

  • Previous experience in addiction treatment or mental health services strongly preferred.

  • Strong communication, interpersonal, and problem-solving skills.

  • Ability to work effectively both independently and as part of a multidisciplinary team.

  • Knowledge of relevant laws, regulations, and ethical standards related to addiction treatment and case management.

  • Valid Ohio Driver's License
