Highland-clarksburg Hospital Inc is seeking a social services professional to provide case management and social work support for patients and families in a hospital setting. You will collaborate with the treatment team, coordinate referrals, and assist with discharge planning and aftercare services to ensure seamless care transitions.
Responsibilities include documenting daily activities, coordinating with schools, courts, and community agencies, and facilitating life skills groups.

Individual provides social services assistance/case management to patients and their families. Works collaboratively with the treatment team in aspects of patient care; links patients with needed aftercare resources/referrals; discharge planning; completes, assists with, and/or schedules needed assessments; makes collateral contacts; examines case situations and presents information to medical staff and other members of the treatment team regarding patient’s family and social background pertinent to diagnosis and treatment. Reports to the Program Manager.

Bachelors of Science or Arts, preferably in social work.
Eligibility for Social Work License preferred
Ability to read and interpret instructions, documents and follow procedures. Ability to write routine reports and speak effectively with patients and their families, employees and public.
Ability to solve practical problems and deal with a variety of variables as needed.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Ability to work flexible hours. Ability to work with children, adolescents or adults experiencing mental health disturbances or dual diagnoses.
