Job summary

RCIL, Utica, NY, is hiring a Case Manager to provide community-based case management for individuals funded by OPWDD. You will coordinate services, monitor budgets, and liaise with support teams by phone, email, or Zoom.

The role requires strong organizational and communication skills, with a preference for a Bachelor/Master of Social Work and at least 2 years of case management experience. RCIL offers rich benefits and a potential 4-day schedule after six months.

Qualifications

  • BSW/MSW preferred; degree in related field considered.
  • College degree in Human Services or related field acceptable.
  • High school diploma or equivalent required.

Responsibilities

Case Manager

Professional Utica, NY, US

Salary Range: $40,040.00 To $44,044.00 Annually

Hourly wage $22.00 - $24.20 ($40,040 - $44,044 a year)

Growth advancement opportunities/career ladder with the first opportunity of a 10% wage increase within 6 months to a year of hire.

  • PTO – vacation (16-24 days a year based on longevity) Sick leave 12 days a year, and 13 paid holidays.

Job Duties:

The successful candidate will be expected to:

  • Work as part of a team to provide case management to individuals receiving services in the community through OPWDD funding (Self Direction program).
  • Support individuals with disabilities to utilize their services and funds to live in the community.
  • Communicate (primarily via phone, email or zoom) with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations.
  • Obtain and maintain required documentation. Review all documentation for accuracy and guidelines adherence.
  • Document all communications.
Education:
  • A Bachelor or Master of Social Work degree is preferred.
  • A college degree in Human Services or related field will be considered.
  • A high school diploma or equivalent is required.
  • Experience providing OPWDD funded services or other case management services may be considered in lieu of higher education.
Knowledge, Skills & Abilities:
  • 2 years of Case Management experience preferred. Higher education will be considered in lieu of experience.
  • Strong customer service and computer skills.
  • Ability to work in a fast-paced environment.
  • Knowledge of Microsoft Excel, Word, and Outlook.
  • Excellent verbal and written communication skills.
  • Strong organizational skills and attention to detail.
