Home Base Inc. is statewide in West Virginia and seeks a Wraparound Facilitator/Case Manager for a full-time position with benefits. You will support families, coordinate services, and lead interdisciplinary planning to help children succeed.

Qualifications

• Bachelor’s degree in a human services field is required.
• Ability to pass NCIC/CIB and APS/CPS background checks.
• Strong written and verbal communication skills.
• Proficiency with Outlook, Word, Excel, and PDF platforms.
• Ability to work independently with minimal supervision.
• Willingness to work evenings and weekends as needed.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide wraparound services to a caseload of up to 15 families.
• Coordinate supports, resources, and services using a strengths-based approach.
• Facilitate team meetings with youth, families, and multidisciplinary partners.
• Collaborate with MCO care managers and community resources.
• Conduct and administer the CANS assessment.
• Monitor service implementation and ensure families receive identified supports.
• Lead planning from engagement through transition and discharge.
• Conduct home visits according to each family’s individualized plan.
• Identify service gaps and collaborate with community providers to address needs.
• Maintain accurate, timely, and compliant documentation.
• Participate in scheduled supervision and coaching.
• Maintain client records in alignment with agency standards and regulatory guidelines.

Physical Requirements

• Ability to walk up and down stairs
• Ability to lift 5–50 pounds
• Ability to kneel, bend, squat, and stand
• Ability to operate a motor vehicle and follow all state traffic laws
