Job summary

The Salvation Army North and Central Illinois Division is seeking a dedicated Residential Services professional to provide comprehensive case management for clients experiencing homelessness and related needs.

Responsibilities include intake review, needs assessment, case plan development, referrals, transit arrangements, and weekly client meetings. Strong documentation, collaboration with outreach, and adherence to program requirements are essential.

Qualifications

  • Bachelor of Science in social work, sociology, criminal justice or related field.
  • In lieu of a degree, five years’ experience in social work or case management will be considered.
  • Minimum of 3 years of homeless or vulnerable population experience with strong documentation.
  • Excellent written and verbal communication; confidentiality and teamwork emphasized.
  • Ability to multi-task, work with diverse backgrounds, and manage budgets.

Responsibilities

  • Provide case management: review intake material, assess needs, develop case plans and ensure compliance.
  • Act as liaison to community resources and arrange client transportation.
  • Ensure access to a case manager via appointments and walk-ins.
  • Meet with clients weekly or as determined; participate in discharge planning.
  • Document client information and progress in HMIS Service Point.
  • Submit statistics to DHS, HUD, and The Salvation Army as scheduled.
  • Attend CIOC meetings and resident house meetings; determine client eligibility.
  • Advocate for clients; be first contact for client-related issues and disputes.
  • Maintain client records ethically and confidentially; uphold Safe Harbor policies.
  • Collaborate with outreach workers; maintain eligibility lists for non-residents.
  • Facilitate outreach and training activities; participate in On-Call rotations for Safe Harbor.
  • Develop Job Leads Listing; maintain professional communication and conflict management.

JOB SUMMARY

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide case management services to clients to include, but not limited to reviewing intake material for accuracy, assessing client needs, case plan development and compliance, act as liaison to community resources, provide referrals to appropriate agencies, arrange for client transportation
  • Ensure that all clients have access to a case manager through appointments and walk ins
  • Establish first contact within 5 days
  • Meet with clients weekly or as otherwise determined
  • Attempt to meet for discharge planning
  • Document client information and progress through HMIS Service Point.
  • Record, track and submit statistical information as scheduled to maintain compliance with DHS, HUD, and The Salvation Army.
  • Attend monthly Continuum of Care (CIOC) meetings as directed
  • Conduct quarterly resident house meetings as directed
  • Determine client eligibility.
  • Act as an advocate for clients when needed
  • Act as first point of contact for client related issues to include:
  • Addressing disputed incident reports
  • Render appropriate resolution with flexibility and discretion
  • Maintain all client records ethically and confidentially.
  • Strive to meet the outcomes of the Continuum of Care grant project.
  • Attend conferences, seminars and staff meetings as directed.
  • In administering Safe Harbor programs, uphold the policies, goals and mission statement of The Salvation Army.
  • Collaborate with outreach workers and maintain eligibility list for non-residents utilizing Day Center Services.
  • Facilitate outreach and training activities for clients by maintaining community partnerships.
  • Participates in rotating ON CALL schedule to provide 24/7 coverage to Safe Harbor operations.
  • Develop Job Leads Listing.
  • Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices.
  • Maintain compliance in The Salvation Army’s “Safe From Harm” Certification.
  • Other duties as assigned by the Residential Services Director or Corps Officer
  • Successfully satisfy the training requirements associated to the position and as required by The Salvation Army.

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES

Education:

Bachelor of Science degree in social work, sociology, criminal justice or another related field. In lieu of a degree, candidates with a minimum of five years’ experience in social work or case management with a social service organization will be considered.

Experience:

Minimum of 3 years of experience working with individuals experiencing homelessness or other vulnerable populations. Demonstrated proficiency in case management, with a strong focus on accurate documentation, timely reporting, detailed recordkeeping, and compliance with program requirements.

Skills/Abilities

  • Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member.
  • Close attention to detail and a high level of confidentiality
  • Ability to multi-task several projects concurrently
  • Must have an interest and ability to work with people of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
  • Ability to establish and implement policies and procedures.
  • Demonstrated awareness of budget resources and cost control, as well as planning and administering budgets.
  • Proficiency in computer software applications, including demonstrated experience in using email, MS Office applications.
  • This individual will only commit resources of The Salvation Army that have been allocated or approved.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position. The individual will keep the designated Supervisor informed of all critical issues relating to their area of responsibility.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • This position may be required to do light to moderate physical work. Sometimes heavier.
  • To successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment including a PC and potentially make public presentations/interactions.
  • The work environment for this position includes an office environment with a low to moderate noise level.
  • This individual must be able to travel to meet the needs of the position. A valid MVR is required.

REPORTING RELATIONSHIPS

  • This position reports to the Residential Service Director and reporting to the Community Service Director and Grants Specialist.
  • In context related to this position’s duties, this individual will interact with a wide range of contacts both internal and external. In these contacts he/she will act as a representative of The Salvation Army and its mission.

PERFORMANCE MEASURES

  • This individual will be evaluated on how effectively the outcomes of this position are achieved. Timeliness and accuracy of accomplishing assigned goals will be reviewed. An evaluation of how effective service is provided. Performance will also be determined by outcome information provided during weekly staff meetings and monthly reports.

This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.
